1. PSBMT Application Form

2. Letter of intent to apply as a diplomate addressed to the President of PSBMT

3. Curriculum Vitae

4. Photocopy of latest PRC ID (front and back)

5. Photocopy of Certificate of Letter of Completion of Hematopoietic Stem Cell Transplantation Training (preferably with inclusive dates or its equivalent, signed by his/her immediate supervisor in HSCT Training)

6. Photocopy of Certificate as Diplomate/Fellow from subspecialty society (eg. PCHTM, PSPH, PSMO, PSPO)

7. Latest 2x2 picture

*Physicians of different specialties and subspecialties

2. Letter of intent to apply as an associate addressed to the President of PSBMT

3. Curriculum Vitae

4. Photocopy of latest PRC ID (front and back)

5. Latest 2x2 picture

REQUIREMENTS FOR APPLICATION FOR AFFILIATE

*Allied Medical Professionals

1. PSBMT Application Form

2. Letter of intent to apply as an affiliate addressed to the President of PSBMT

3. Curriculum Vitae

4. Photocopy of latest PRC ID (front and back)

5. Latest 2x2 picture
